Integrated Care Referral Program
NC State’s Integrated Care Referral Program connects Campus Health, the Counseling Center, and Wellness and Recreation to support student well-being through coordinated care. The program allows providers to refer students to evidence-informed services that complement clinical care and support sustainable behavior change.
Through this referral pathway, students may be referred to Wellness Coaching and/or Exercise is Medicine® by their on-campus provider based on individual needs, readiness, and health considerations.
We recognize that movement is more than exercise; it’s a powerful tool for reducing stress, improving focus, and enhancing quality of life. Our goal is to help you move more, feel better, and build healthy habits that last. Exercise is Medicine® at NC State promotes physical activity as a vital part of overall well-being. Supporting physical health, mental clarity, and daily energy are the program’s primary goals.

Programs
Wellness Coaching
Through this program, participants learn practical strategies and skills to explore wellness best practices tailored to their current lifestyle, while also having the opportunity to expand and connect additional goals as they naturally emerge.
More Info
Wellness Coaching provides individualized, student-centered support focused on behavior change, goal setting, and skill development. Coaching sessions are collaborative and designed to help students build habits that support long-term well-being.
Students may be referred to Wellness Coaching for additional support with resilience, sleep, nutrition, chronic disease management, or building consistent, balanced routines.
Wellness Coaching complements medical and mental health care by focusing on daily behaviors that influence overall health.
Session Details
4 sessions to build healthy lifestyle practices:
- An initial discovery session to identify wellness goal(s) and assess readiness for behavior change.
- Three follow-up sessions focused on refining goals and developing personalized, sustainable strategies.
- Access to a wellness resource library featuring both on- and off-campus resources to support different learning preferences.
- Ongoing access to a certified wellness coach for guidance and support throughout your wellness journey.
Exercise is Medicine® – On Campus
Through this program, participants learn basic exercise fundamentals and practical ways to incorporate movement into their daily routines in ways that feel realistic and sustainable.
- Students must be referred to this program by a Campus Health or Counseling Center provider.

Exercise is Medicine® supports students in safely increasing physical activity through individualized guidance and connection to campus resources. Through this program, participants learn basic exercise fundamentals and practical ways to incorporate movement into their daily routines in ways that feel realistic and sustainable.
As part of the national Exercise is Medicine on Campus (EIM-OC) initiative, NC State works to make movement a visible part of campus culture, assess physical activity during visits to Campus Health and the Counseling Center, and connect students with wellness and fitness resources across campus.
Session Details
4 sessions to build your movement momentum that include:
- Introductory orientation to Wellness and Recreation Center.
- 3 follow-up sessions to focus on foundational movement and behavior change principles to help establish a movement routine that supports your life at NC State and beyond.
- An optional action planning session to strategize ways to maintain movement in your daily life.
- Access to a resource library of movement-based information.
- Access to a certified personal trainer to help support your physical activity goals.

How do I get started with these programs?
- This is a referral-based program that requires your provider from Campus Health or the Counseling Center to submit on your behalf. A referral to either program is the first step to getting started.
How do I get referred to the Integrated Care program?
- Referrals are based on a set list of eligibility criteria developed by care providers, staff members, the Integrated Care Steering Committee, and the American College of Sports Medicine.
- Staff and providers will determine the most appropriate program and may make referrals to other departments or programs based on the initial consultation and readiness for change.
How do I know if the Integrated Care program is right for me?
- The Integrated Care Referral Program is not a replacement for seeking emergency or medical attention.
- This program is an excellent fit for students who are looking to get started or restart a movement or wellness program that supports their well-being holistically.
